My Heart Care Services LLC is seeking a driven Sales Representative with proven experience in home care, hospice, hospital discharge planning, and physician referral networks. This is a high-impact, success-based role designed for a professional who already has strong connections in healthcare and can convert leads into client admissions.

🔑 Key Responsibilities

Develop and maintain referral relationships with physicians, hospital discharge managers, hospice providers, and rehabilitation centers.

Present and promote My Heart Care Services’ non-medical home care solutions to referral partners and families.

Manage the full sales cycle – from prospecting to referral to client admission.

Collaborate with agency leadership to refine growth strategies and outreach programs.

Meet and exceed referral and admission targets, with performance tied directly to commission growth.

✅ Required Qualifications

Experience in home care, hospice, or healthcare sales is mandatory.

Established relationships with discharge planners, case managers, physicians, and senior living communities in DFW.

Strong ability to educate healthcare professionals on service benefits and coordinate patient transitions.

Self-motivated, goal-oriented, and able to thrive in a success-based environment.

3+ years of direct healthcare sales experience (home care, hospice, or home health).

Knowledge of Medicaid, private pay, and long-term care referral processes.

Strong negotiation and presentation skills.
